CCK is the major humoral mediator of meal-stimulated pancreatic enzyme secretion. The circulating concentration of CCK increases with a meal. Experiments using highly specific CCK receptor antagonists have demonstrated that pancreatic enzyme secretion with a meal is largely mediated by CCK. CCK is released from the upper small intestinal mucosa by digestion products of fat and protein and, to a small extent, by starch digestion products. Experimental data suggest that CCK activates afferent neurons in the duodenal mucosa. These afferent neurons mediate an enteropancreatic reflex that causes pancreatic enzyme secretion.
