The majority of the indices that have been developed to measure disease activity in UC are nonspecific and have not been prospectively validated. Indices may be purely clinical, endoscopic, or histologic, and some combine clinical and endoscopic assessment. Additionally, scores on these indices are often based on various symptoms and signs for which there is no unifed definition. The Truelove and Witts classification of UC is based on the frequency of bowel movements, the amount of blood, the presence of fever and tachycardia, anemia, and the erythrocyte sedimentation rate, all of which are clinical measurements; patients are classified as having mild, moderate, and severe disease. Although it is the oldest index, it is still one of the most commonly used because of its simplicity. The Ulcerative Colitis Disease Activity Index or Sutherland Index gives a numerical score that takes into account not only the clinical measures of stool frequency, rectal bleeding, and physician’s global assessment, but also incorporates sigmoidoscopic appearance; it does not, however, include a histologic subscore. Patients are considered to have fulminant colitis when they have a toxic appearance, a fever higher than 101°F, tachycardia, abdominal distention, signs of peritonitis, and leukocytosis. Although these patients may also have toxic megacolon (radiologic measurement of the transverse colon >6 cm), this is not implicit in the definition of fulminant colitis.
